In order to schedule with us:
1. If you have an insurance card, have it available when you call to schedule
2. Dial (505) 768-7507 for general scheduling for both orthopaedic and spine, or (505) 724-3232 to schedule an appointment for a work-related injury.
3. Provide us with some basic info about yourself and the reason you're requesting a visit. Our schedulers will book the first available appointment with a Physician who can tend to your needs. The information our schedulers will need for each patient includes their:
a. Patient Name
b. Date of Birth
c. Social Security Number
d. Address and Phone Number
e. Name of Primary Care Physician or Referring Physician
f. Emergency Contact Information
g. E-mail address
h. Date of Injury or Pain Onset
4. Please plan to arrive 30 minutes before your appointment time on
the day of your visit to allow ample time for parking and check-in
