Release of Information

In compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA), New Mexico Orthopaedic Associates requires your written consent before disclosing any personal health information.  This request is valid for 90 days from the date of signature.  Your consent to share this information may be withdrawn in writing at any time, so long as such documents are specific as to information covered as well as dated and signed.  Please note: Any information shared pursuant of this consent may be subject to re-disclosure by the recipient and thus may no longer be protected by the HIPAA privacy rule.

To formally request release of your records, in part or whole, please fill out a Permission for Release of Information form in its entirety.  Be sure to sign and date the document and submit your request to the New Mexico Orthopaedic Associates Medical Records Department.  Signed requests are accepted through postal mail or in-person, when necessary, but please keep in mind that both methods are less efficient and somewhat time consuming, and thus are not recommended.  To expidite your request and streamline the process, please submit your completed release form(s) via e-mail or fax to (505) 724-4401. 

 

Please allow 48-72 hours from time of receipt for our staff to process your request.  Identification will be required for individuals picking up records.  Department hours are Monday through Friday from 9am to 4pm, MST.

 

There will be a charge for a personal copy of medical records.  The cost is $2/per page for the first ten pages and 20¢/per page for each additional page, as allowed by HIPAA regulations.  Tax, postage and handling are additional.  You will be invoiced directly for this service by HealthPort, whom New Mexico Orthopaedics contracts with to facilitate release of medical information requests.
