On the day of you appointment, please make sure to bring each of the following with you to speed and simplify the check-in process.  

1. Insurance Card
2. X-rays or other imaging taken of your current ailment
3. Photo ID
4. A list of all medications (including herbal and over the counter) you are currently taking
5. If you are under 18, you must be accompanied by a parent or legal guardian to your visit
6. Bring your referall with you, or have your doctor fax it to us in advance at (505) 224-8235

Also, please take the time to complete and bring along the New Patient Form if you have the opportunity.  We request that you arrive 30 minutes before your appointment time on the day of your visit to allow ample time for parking and check-in processes.  For a map to our facility, click here.
